This document will also allow the parent to admit their child to certain medical facilities and access medical records under HIPAA.
- Living Will/Advance Healthcare Directive: This document also allows parents to make certain medical decisions on behalf of their child, though these decisions generally involve situations wherein the child requires certain end-of-life medical decisions, such as whether he or she should be placed on a ventilator, etc.
